What To Expect At Your Appointment:
When you come in, expect our A game. We will conduct a full body movement assessment and ask for your medical and athletic history. You will have our undivided attention which may need to be directed at you from all angles, so dress comfortably.
Please understand we’ll need access to the part of you that hurts. Generally speaking, patients prefer to wear workout gear like capris, leggings or shorts. If we’ll be working on your upper body, a sports bra and tank top are ideal. As a rule of thumb, if we need to treat it, you’ll need to show it. Your feet may also be bare, so please consider that in advance, for our sake and yours.
We want to dedicate your entire appointment time to your care, so be ready to roll when you show up, or arrive early to change in our locker rooms. All patients have access to the gym, locker rooms, showers and towel service. Locks for lockers are provided. Pretty sweet.
You can expect to leave your evaluation with a solid understanding of the road ahead and the plan of care your therapist has mapped out for you. You will also receive a homework regimen of exercises tailored to your needs and goals to empower you to self-manage your pain and further your recovery. Before you leave, your therapist will walk you through the exercises in detail. But you’ll also have access to an app and online portal that outlines everything you should be doing. (We don’t expect you to remember everything from the get-go.)

If you haven’t already sent over your insurance card and prescription if you have one, please do it now or bring them with you.
To save time (especially if you tend to run late or have chronic issues with the MTA), please click the shiny buttons below to download the oh-so-cleverly entitled new patient form and credit card authorization. Print, sign and bring them with you. Or fill them in and email them back; we’ll have printed copies ready for you to John Hancock at your appointment. Please do not edit the forms in your browser; download them first. (We’re going out on a limb and assuming you’d rather not share your personal info with the world.) We really appreciate you taking care of the forms in advance so we can focus on your body and how it moves instead of paperwork.
Lastly, if you’re a runner or athlete and suspect your gear or footwear is causing issues, please bring those bad boys with you so we can check them out firsthand.

We’ve carved out time just for you, and bailing on your appointment in less than 24 hours results in a cancellation fee, even if you let us down gently. That’s a bummer for everyone and more importantly, delays you getting better. So let’s avoid all of that and get you in here.
