We want your entire appointment time to be about YOU, not forms. So please click the shiny buttons below to download our new patient form and credit card authorization. Either fill them out and bring them with you, or email them back to FixMe@ClutchPT.com. We’ll have printed copies ready for you to John Hancock at your appointment.


TIPS FOR CRUSHING YOUR APPOINTMENT:

  • Know where you’re going. We’re located on the 5th Floor of 244 East 84th Street, between Second and Third Avenue.

  • Be ready to roll when you show up, or arrive early enough to change.

  • You will have our undivided attention, which may need to be directed at you from all angles, so dress comfortably. Athletic wear is a solid choice. As a rule of thumb, if we need to treat it, you’ll need to show it.  

  • Under normal circumstances, our patients have access to the gym, locker rooms, showers and towel service. However, in this surreal time, the locker rooms are closed. You will have access to restrooms, though.

  • If you suspect your footwear is contributing to your pain or issues, please bring those bad boys with you so we can check them out firsthand. 

  • For virtual PT sessions via Zoom, it’s helpful if you’re able to have someone else there to move your device around for you, so we can see multiple angles while you focus on moving. But don’t sweat it if that’s not possible! After your appointment, you’ll receive a link to the recording which you can download and keep for reference. We strongly recommend that option! A whole library of our wisdom and expertise at your fingertips? Yes, please.

  • Do your homework. Help us help you. Before you leave, your PT will walk you through the exercises you need to do in detail. You’ll also have access to an online portal that outlines everything you should be doing. We’ll know if you do or not!

  • If you haven’t already sent over your insurance card and prescription if you have one, please do it now or bring them with you.

  • Bailing on your appointment in less than 24 hours results in a cancellation fee, even if you let us down gently. That’s a bummer for everyone and delays you getting better. So let’s avoid that. Call us ASAP if you need to re-schedule.

  • We love questions. Bring them all.
